Job Summary
The Care Coordinator for the Suicide Prevention Team works with individuals who have experienced suicidal ideation or suicide attempts, typically following discharge from emergency departments or inpatient psychiatric facilities. This role focuses on rapid follow-up, care coordination, and transition planning to support safety and stability during high-risk periods, particularly for individuals with serious mental illness and/or substance use disorders.
The Care Coordinator for the Suicide Prevention Team works with individuals who have experienced suicidal ideation or suicide attempts, typically following discharge from emergency departments or inpatient psychiatric facilities. This role focuses on rapid follow-up, care coordination, and transition planning to support safety and stability during high-risk periods, particularly for individuals with serious mental illness and/or substance use disorders.
The Care Coordinator develops individualized care plans, coordinates services, and ensures timely follow-up after discharge to reduce risk and support continuity of care across behavioral health systems.
Responsibilities
- Complete initial and ongoing assessments, including suicide risk, substance use, mental status, medical history, living situation, supports, strengths, barriers, and medications.
- Conduct outreach to crisis stabilization units, inpatient psychiatric facilities, emergency departments, and community partners to engage eligible individuals.
- Conduct suicide risk screenings and complete comprehensive risk and biopsychosocial assessments at time of referral.
- Develop individualized care and safety plans in collaboration with individuals served, families, and providers.
- Provide safety planning and counseling on access to lethal means.
- Maintain frequent, proactive contact based on level of suicide risk, including follow-up within 24 hours of missed appointments.
- Coordinate transition of care, including warm hand-offs to ensure connection to ongoing services.
- Support stabilization through outreach, care coordination, and ongoing engagement.
- Monitor progress, document outcomes, and participate in care plan or treatment plan reviews.
- Advocate for and coordinate access to needed services and community resources.
- Maintain regular communication with providers and support continuity during and after treatment.
- Provide outreach to community providers during crisis points to promote program services and collaboration.
- Identify and report concerns related to risk, trauma, abuse, neglect, or abandonment in accordance with policy and regulations.

- Transforms brief notes or session conversations into complete, clinically appropriate documentation
- Supports accurate, compliant documentation while allowing staff to review and finalize notes
- Provides helpful insights from sessions to support care planning and continuity
- Accessible on desktop or mobile for flexibility across different work settings
Qualifications
- Bachelors degree in Social Work or a related Human Services field from an accredited university or college required.
- Minimum of two years of experience in human services or behavioral health required; experience working with adults with mental illness preferred.
- Must demonstrate proficiency in MHRC EHR System within three months of employment. Proficiency in Microsoft Office and email are required.
- Strong communication and interpersonal skills; ability to work effectively with individuals, families, providers, and team members.
Position Details
This is a Full Time Days position: Monday through Friday, 8:00am to 4:30pm (may work occasional evenings).

Mental Health Resource Center, Inc. (MHRC) is a nonprofit organization that provides a wide range of behavioral health and social services to the community. Included in MHRC’s service array are 24-hour emergency services and inpatient psychiatric services for children, adolescents, and adults. Outpatient services include medication management, care coordination, case management, intensive case management team services, counseling, psychosocial rehabilitation, an adult therapeutic family program, state hospital liaison services, mental health court, multidisciplinary forensic teams (MFT), jail-based diversion services, a Co-Responder Program with the Jacksonville Sheriff’s Office, the Link-to-Life suicide prevention program, and services that assist individuals in obtaining benefits such as Social Security Income, Social Security Disability Income, Medicaid, food stamps, and housing. In addition, UF Health Jacksonville contracts with MHRC to manage the hospital’s adult inpatient psychiatric unit.
MHRC also operates eight Florida Assertive Community Treatment (FACT) programs for adults with severe and persistent mental illness, located in Clearwater, Gainesville, Jacksonville, Kissimmee, Rockledge, Tampa, and Winter Haven, as well as a FACT Lite program providing Linking, Advocating, Treating, Transitioning, Empowering & Recovery Support (LATTERS) services in Jacksonville. The two FACT teams located in Jacksonville serve residents of Duval, Clay, and Nassau counties.
