Job Summary
The CSC Care Coordinator – Patient Access and Support ensures continuity of CSC services across CSC North and CSC South by providing adaptable coverage based on daily operational needs. This role supports timely access to intake/assessment, coordinates outpatient psychiatric medical services activities (e.g., scheduling, follow-up, client outreach), and strengthens care navigation, including community resource linkage, client engagement, and discharge bridge appointments/transition support.
This role enhances CSC operational stability by reinforcing timely access to intake assessments, care coordination, and client engagement across both sites, reducing service disruption and minimizing reliance on last-minute coverage from other departments.
Responsibilities

- Provides initial screenings and assessments for individuals requesting CSC services, including telephone, scheduled, and walk‑in contacts, in alignment with CSC intake workflows and policies, under supervision of a LIP or Clinical Manager as appropriate.
- Completes intake/assessment documentation (e.g., initial assessments, risk screens, social determinants of health, tobacco use, and other required screening tools) and coordinates warm handoffs to appropriate CSC and community services.
- Supports determination of service needs and level of care in coordination with Intake Clinicians and CSC leadership, ensuring timely linkage to psychiatric, therapy, and case management services.
2. Medical services care coordination
- Works closely with Psychiatric Nurse Practitioners and Psychiatrists at CSC North and South to support individuals receiving outpatient psychiatric medical services, including completing initial and ongoing assessments related to living situation, support system, medical status, medications, and social determinants of health.
- Develops and implements person-centered treatment and service coordination plans in collaboration with individuals served, families, and providers, monitoring progress and supporting stabilization of mental health symptoms through regular contacts and coordination.
- Obtains and documents vital signs, supports medication management appointments, ensures medication and medical records are current, and facilitates referrals and care coordination across disciplines and community providers.
3. Care Navigation, Community Resources, and Client Engagement
- Provide care navigation support to strengthen continuity of care and client engagement across both CSC locations.
- Link clients to community resources (e.g., benefits, primary care, housing/resources, transportation supports, and other social service supports as appropriate).
- Conduct outreach to improve engagement and follow-through (appointment reminders, follow-up calls, barrier problem-solving, and re-engagement efforts).
- Support continuity around transitions of care by helping coordinate discharge bridge appointments and follow-up planning for clients transitioning from inpatient units or other higher levels of care into CSC services.
4. Operational Flex Support Across CSC Roles
- Flex between CSC operational needs on a day-to-day basis, including coverage of functions that may overlap intake support, medical services coordination, and care navigation.
- Coordinate daily priorities with CSC managers and team leads to determine where coverage is most urgently needed.
- Support a “continuity mindset” by anticipating gaps and proactively addressing coverage risks (e.g., backlogs, scheduling disruptions, increased no-show risk).
Qualifications
- In order to be considered, candidates must have a Bachelor’s degree in social work or a related human services field from an accredited university or college (a related Human Services field is defined as one in which 30 hours of coursework includes the study of human behavior and development) required.
- Minimum of one year experience working with adults with serious mental illness (SMI) required.
- Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
- Must be able to communicate and write English clearly, legibly and correctly at the college graduate level in order to make entries in the records of persons served. Documentation, paperwork and system entries must meet internal and external guidelines for content, accuracy and timeliness.
- Excellent customer service skills are essential and this individual must be able to interact appropriately with internal and external customers, including individuals served, families, caregivers, community service providers, supervisory staff and other department professionals.

Mental Health Resource Center, Inc. (MHRC) is a nonprofit organization that provides a wide range of behavioral health and social services to the community. Included in MHRC’s service array are 24-hour emergency services and inpatient psychiatric services for children, adolescents, and adults. Outpatient services include medication management, care coordination, case management, intensive case management team services, counseling, psychosocial rehabilitation, an adult therapeutic family program, state hospital liaison services, mental health court, multidisciplinary forensic teams (MFT), jail-based diversion services, a Co-Responder Program with the Jacksonville Sheriff’s Office, the Link-to-Life suicide prevention program, and services that assist individuals in obtaining benefits such as Social Security Income, Social Security Disability Income, Medicaid, food stamps, and housing. In addition, UF Health Jacksonville contracts with MHRC to manage the hospital’s adult inpatient psychiatric unit.
MHRC also operates eight Florida Assertive Community Treatment (FACT) programs for adults with severe and persistent mental illness, located in Clearwater, Gainesville, Jacksonville, Kissimmee, Rockledge, Tampa, and Winter Haven, as well as a FACT Lite program providing Linking, Advocating, Treating, Transitioning, Empowering & Recovery Support (LATTERS) services in Jacksonville. The two FACT teams located in Jacksonville serve residents of Duval, Clay, and Nassau counties.
